Start Small Campbell Grant Application

    Have these files ready to upload before you begin completing the form. The application does not auto-save and must be completed in one sitting. Be sure to rename all templates with the business name before submitting.

  2. Complete all the required fields in the form below and be sure to click Submit. Staff will preview your submission to ensure a complete application is received. Incomplete applications will not be presented to the Review Committee for consideration.

  4. Name of business owner completing the form. All other owners of the business should be listed under "Additional Business Owners."

  5. As listed on your Campbell County Business License. If operating under a fictitious or trade name, please list both the legal name and operating name.

  6. Street address in Campbell County where the business is/will be operating. (Should match information on Campbell County business license application).

  7. Applicant's Email Address

  8. Includes members of a partnership and officers of a corporation or limited-liability company (LLC). Sole proprietors may enter "None."

  9. Number of Years in Business*

    The Start Small Campbell Grant Program is available to start-up business (less than 3 years in operation) and businesses opening their first location in Campbell County.

  14. Upload Income Statement from bookkeeping software (Business is already open) or 12 month Budget Planning Worksheet (if your business has not yet opened).

  15. Provide a statement projecting your income in Year 2 and Year 3 to go along with the 12 month income statement (See Financial Template)

    Applicants are required to attend at least two qualifying workshops/programs within 12 months of their application date. Classes are offered throughout the year. Contact the SBDC-Lynchburg Region, Spark Innovation Center, or the Center for Entrepreneurship to enroll.

  19. Business Plan Mentor Review is Complete*
  20. Please enter the name of your business plan reviewer. Approved reviewers are: Autumn Evans (Spark Innovation Center), Stephanie Keener (Lynchburg SBDC), Jordan Reeves (Lynchburg SBDC), and Maria Niechwiadowicz (Lynchburg SBDC).

  21. You may choose to include additional information you feel would strengthen your application. If you have additional documents to send via email, list the names of the documents with a brief description here, so staff can ensure all additional documentation has been received.
